You agree to the following terms and conditions of Create physio and Pilates when booking an appointment:
Payment for sessions is done in advance or on the day by bank transfer.
All appointments are self pay, I do not directly bill any insurance companies. You may be able to claim some of the session costs back from your health insurance provider but you must speak to them and get their confirmation. If required I can issue a receipt after your session.
If you need to cancel a session you must give at least 24 hours notice. Sessions cancelled within 24 hours of the appointment time will be charged at the full rate. Cancellations can be made by phone or email.
